SS-31 discussions changed after elamipretide received an FDA approval. The milestone is real: Forzinity was granted accelerated approval in September 2025 to improve muscle strength in adults and children with Barth syndrome who weigh at least 30 kg. The approval is not a general endorsement of mitochondrial peptides for fatigue.
Reddit communities for ME/CFS and long COVID describe both improvement and worsening. Those conditions are heterogeneous, spontaneous fluctuation is common, and increased activity can lead to delayed post-exertional malaise that makes an initially energetic day misleading.
What favorable and unfavorable posts describe
Favorable reports mention clearer thinking, easier walking, more daily energy, improved exercise tolerance, or reduced recovery time. Some users describe an early boost that later fades. Others report no benefit or feel worse after increasing activity because the first change seemed encouraging.
Unwanted reports include injection-site reactions, fatigue, headache, nausea, sleep disturbance, or post-exertional worsening. Product identity is a major unknown when users obtain material outside the approved Forzinity supply chain.
The approval is narrow and conditional
Forzinity’s accelerated approval is for Barth syndrome and is based on improvement in a surrogate or intermediate measure considered reasonably likely to predict clinical benefit. FDA requires a confirmatory trial. The approved label, manufacturing, and population are part of that decision.
A vial marketed as SS-31 is not automatically Forzinity. It may differ in salt form, content, impurities, sterility, packaging, and instructions. Nor does the Barth syndrome indication validate use for long COVID, ME/CFS, healthy aging, or athletic performance.
What broader trials found
Elamipretide has been studied in primary mitochondrial myopathy. In the MMPOWER-3 randomized trial, it did not significantly improve the primary six-minute walk or fatigue outcomes compared with placebo. A trial during ST-elevation myocardial infarction also did not meet its primary benefit endpoint.
Negative trials do not show that the molecule has no effect in every disease. They do show why a compelling mitochondrial mechanism and a success in one rare disease cannot be generalized to all fatigue states.
Product and indication are inseparable
“FDA-approved peptide” is incomplete. The accurate statement names the approved product, population, indication, and ongoing confirmatory requirement.
How to track fatigue without overexertion
- Use a stable, submaximal daily task instead of a maximal test.
- Track function and symptoms for at least 48 hours after increased activity.
- Separate cognitive function, orthostatic symptoms, pain, sleep, and physical endurance.
- Keep pacing, medications, infections, menstrual cycle, and rehabilitation visible.
- Do not increase activity solely to prove that a perceived energy change is real.
How to make your tracking more useful
Strong personal logs begin with a short baseline and keep the same measurements long enough for a pattern to emerge. Peptify can connect dates, context, and observations locally on your device so the record is more useful than memory alone.
- A safe submaximal function measure, daily steps or activity, and symptoms 24 to 48 hours later.
- Fatigue, cognition, orthostatic symptoms, pain, sleep, and time spent upright.
- Injection-site reactions, headache, nausea, medication changes, and exact product details.

Questions to discuss with a healthcare practitioner
- Does the person meet the approved Barth syndrome indication for Forzinity, or is the proposed use entirely different?
- Could anemia, endocrine disease, sleep disorder, autonomic dysfunction, medication effects, or another cause explain fatigue?
- How should post-exertional worsening be prevented and monitored?
